On June 18, 2023, the submersible Titan embarked on a dive to the wreck of the Titanic, which lies at a depth of 3,800 meters about 600 kilometers from Newfoundland. The five-person crew lost contact with the support vessel just hours later.

After an extensive search, on June 22, it was announced that the submersible had imploded, killing all on board. While several potential causes were identified immediately after the tragedy, new investigative findings have emerged three years later, shedding light on additional factors.

During the search, concerns had already been raised about the safety of the Titan and the negligence of OceanGate. The company's lead specialist repeatedly warned director Stockton Rush about safety issues, but these warnings were dismissed. The latest investigation provides further details on how the disaster unfolded.
