Blue Origin narrows in on cause of catastrophic rocket accident
Blue Origin CEO Dave Limp says the anomaly that destroyed the New Glenn rocket in May originated in the main oxygen valve of one BE-4 engine, but the root cause remains unidentified. The company plans small valve modifications and aims to return to flight by year's end.

Blue Origin has released its first detailed update on the catastrophic loss of its New Glenn rocket and much of the launch pad infrastructure at Cape Canaveral, Florida, in May. Chief executive Dave Limp said investigators are still examining the anomaly that occurred during a hot-fire test on the evening of May 28, 2026. He confirmed for the first time that the problem began in one of the seven BE-4 engines powering the vehicle's first stage.
According to Limp, the origin of the failure was traced to the main oxygen valve on one of the engines, with hardware recovery and inspections later confirming the finding. Extensive component-level tests and engine hot-fires have been carried out to better understand the failure mode and develop mitigation measures. Nevertheless, the executive's statement suggests that a definitive root cause has not yet been established.
Limp said the path forward is clear, with a target of returning New Glenn to flight before the end of the year. The company is making small modifications to the valve that can be quickly retrofitted to existing engines. Updated hardware is expected to be ready by the end of this month, while the remaining fault-tree analysis continues.
The update comes ten weeks after the hot-fire incident. In parallel, Blue Origin has been rapidly clearing debris around the expansive Launch Complex 36-A and starting to rebuild the launch tower. Multiple sources say the company is working with a sense of urgency and sparing little expense in its effort to resume launches of the heavy-lift rocket.
