Antidepressants and antipsychotics could serve as alternatives to opioids, study finds

A new study suggests that various non-opioid medications, including antidepressants and antipsychotics, may serve as effective alternatives to powerful opioids for pain relief in emergency departments.

Researchers from the University of California, San Francisco (UCSF) have published a review paper evaluating non-opioid medications available in the emergency department at San Francisco General Hospital. The goal was to create a targeted list for common pain conditions to expand physicians' treatment options.

Opioids have a strong record of pain relief, but excessive prescribing without regard for addiction risks contributed to the US opioid crisis that began in the 1990s. The authors stress that opioids still have a role in medicine, but alternatives can help reduce long-term risks such as addiction and overdose.

The study analyzed common pain types seen in emergency departments: abdominal, back, chest, fracture, and headache. Common medications like acetaminophen and ibuprofen showed potential for all types. However, some drugs had more specific applications: ketamine for chest pain, an SNRI antidepressant for back pain, and several antipsychotics for headache and abdominal pain.

Psychotropic medications have long been used for pain, and their mechanisms involve neurotransmitters also linked to mood, such as dopamine, serotonin, norepinephrine, and glutamate. Researchers explain that these drugs help regulate the nervous system and reduce hypersensitivity often seen in chronic pain.

Dr. Kathy LeSaint, co-author, emphasizes personalized treatment: doctors should discuss patients' prior opioid experiences and tailor regimens. It is also important to reassure patients that prescribing psychiatric medications does not imply the pain is psychosomatic, but rather reflects the overlap between pain mechanisms and emotional processing.
